Job summary

A fantastic opportunity has arisen to join the Pershore and Upton Neighbourhood Team . We are looking for a newly qualified/experienced occupational therapist who is keen to deliver high quality care in the community setting. You will be part of an integrated team of therapists and nurses who provide essential knowledge and skills to support patient discharges, prevent unnecessary hospital admissions and keep people safe in their own homes.

Your caseload will be varied with an opportunity to see patients with a variety of presenting conditions allowing you to develop and improve existing problem-solving skills. We work closely with GPs, developing relationships with other primary care services, utilising skills of our MDT colleagues to manage complex patients ensuring services are responsive and patient centred.

You will have opportunities for further training and development within the role. You will be joining an enthusiastic, supportive, and friendly team where you will receive support and development through in-service training, regular appraisals, a personal development plan and clinical supervision.

Main duties of the job

The role is Monday to Friday and current working hours are 8.30 to 16.30, we would welcome further discussions if required around days and hours of working due to the nature of this role and the flexibility that the team can provide .

The role will involve you:

Undertaking occupational therapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity and leisure.

Assessing clients: in relation to their equipment and specialist equipment needs / for minor adaptations eg rails, half steps.

Working with clients to identify Occupational Therapy goals as part of the overall care plan.

Monitoring and evaluating treatment to measure progress and ensure effectiveness of the intervention.

Maintaining contemporaneous clinical notes to professional minimum standards of the College of Occupational Therapy and in accordance with Trust guidelines.
