We are seeking an enthusiastic and passionate Occupational Therapist to join our growing Progression Team within Mental Health and Learning Disabilities Commissioning.
This role involves working with individuals in commissioned placements to deliver a high-quality, person-centred occupational therapy service that promotes health, wellbeing, and improved quality of life. The successful candidate will undertake commissioning reviews alongside occupational therapy assessments, ensuring that care packages remain aligned to individual needs.
Main duties of the job
In this role, you will deliver a high-quality, person-centred occupational therapy service to individuals within Mental Health and Learning Disabilities commissioned placements, supporting them to live meaningful lives. You will carry out commissioning reviews alongside occupational therapy assessments, ensuring care packages remain appropriate, effective, and outcome-focused, while regularly monitoring and reviewing needs and eligibility.
You will manage an autonomous caseload, including individuals with complex needs, completing assessments, developing tailored care plans, implementing interventions, and evaluating outcomes. Using your clinical judgement, you will adapt plans to promote independence, recovery, and improved quality of life.
Working as part of a multidisciplinary team, you will contribute to rehabilitation and reablement across a variety of settings, including homes, community environments, and hospital settings. You will maintain high professional standards, ensuring all practice aligns with relevant policies, while producing accurate, timely documentation and working both independently and collaboratively to meet service objectives.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
