Job overview

To support the provision of Health and Social Care to the Older Adults of Bridgend borough who are experiencing a mental health issue within the context of a multi-disciplinary, multi-agency team and support the delivery of evidence based mental health care to the individuals in a community setting.

The post holder will deliver specific components of the individuals' care plans are directed by the Registered Nurse and/or care co-ordinator.

The post holder will be required to work without direct supervision for all or most of their shift.

The post holder will be expected to assist with the organisation and facilitation of monitoring the physical and mental health needs and may run or assist with therapeutic groups.

Main duties of the job

  • Works as part of the multidisciplinary team delivering care to patients/service users experiencing mental health needs in the community/home setting or inpatient services, with support and supervision provided by a registered healthcare professional.
  • Undertakes or supports personal care activities, daily living skills/tasks and clinical care duties with minimal support and supervision as delegated by a registered healthcare professional in accordance with the HEIW All Wales Guidelines for Delegation.
  • Supports the enhanced observation of patients who present as a high risk to themselves or others due to a mental health, cognitive or clinical condition.
  • Ensures the registered healthcare professional is regularly updated on the patients/client’s condition and progress.
  • Ensures the working environment is safe and hygienic following infection prevention and control policies and processes.
  • Contributes to the teaching and supervision of students and junior staff
  • Delivers role in accordance with Health Board/Trust Values and Code of Conduct for HCSW’s in Wales including maintaining physical, procedural and relational security.
