Job overview

We are looking for forward thinking, passionate Nursing Associates to support the management of patients within the Bury Town Locality. You will provide nursing treatments under the delegation of a district nurse and work to the full scope of your registration.

As nurse associate in the Integrated Neighbourhood team you will be responsible for providing safe, effective nursing activities in peoples own homes, care homes and community clinics. You will work closely with all the members of the INT to achieve the best long-term outcomes for all your patients.

Main duties of the job

The role requires you to be able to keep accurate clinical records and have good clerical/ administrative skills and therefore experience of clinical recording systems, ideally SystmOne will be beneficial.

Your day will include-

Gathering and collating information, planning and delivering healthcare for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.

Working within the integrated team to facilitate early discharge from hospital.

Working within the integrated team to prevent unnecessary admission to hospital.

Working with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
