Job Summary

As a registered Nursing Associate, in the Bladder Bowel and Pelvic Heath (BBPH) team, the postholder will be required to work as part of a multidisciplinary team delivering care to patients with support and supervision provided by a registered nurse or physiotherapist. The postholder is required to carry out a range of patient care tasks and duties to enable the team to provide an effective and efficient service within community settings, as well as support the continence project in care homes and the product service.

The role of the Nursing Associate as an accountable professional is to promote health and prevent ill health through the provision and monitoring of care whilst working in teams; contributing to integrated care and improving the safety of quality of care.

Main duties of the job

  • Provide compassionate, safe and effective evidence-based care and support to people in a range of care settings
  • Deliver high quality, compassionate care with a focus on promoting health and independence
  • Work as part of a designated clinical and care team providing and monitoring care that focuses on the direct needs of the individual
  • Maintain a current working knowledge of other providers’ resources and referral systems to ensure individuals needs are met, within parameters of practice
  • Work independently in line with the individual’s defined plan of care, within the parameters of practice of the Nursing Associate role, accessing clinical and care advice when needed
  • Implement individualised care plans ensuring all care provided adheres to policies and procedures.
  • Work in partnership with all members of the multi professional health care team both within and external to the Trust to ensure continuity of care for the patient
  • Promote and develop the use of current evidence to inform the delivery and promote the development of clinically effective care.
  • Communicate effectively with patients, relatives and other members of the care team to ensure a shared understanding of the patient’s care.
  • Maintain accurate, legible and timely records of patient care and information shared
  • Recognise and report any situations, behaviours or errors that could result in poor care outcomes

Workforce

  • Manage a group of patients, prioritising their needs in conjunction with those of the clinical area, to ensure safe and effective care
  • Assist in the delivery of complex care as prescribed by the Registered clinicians·
  • Appropriately delegate care to other members of the team, recognising that they remain the accountable professional for ensuring that any care delegated is completed
  • Participate in the induction and orientation of newly appointed staff and learners to the Clinical area
  • Provide supervision to newly qualified staff and learners, promoting an effective learning environment
  • Support service development and the embedding of the Nursing Associate role by promoting and sharing information on the role / scope of practice
  • Demonstrate behaviours which support equality and value diversity

  • Exercise personal duty of care in the safe use and the appropriate storage of equipment (personal equipment such as laptops should be securely stored for safekeeping of patient information)
  • Have an understanding of clinical area resources and the appropriate use of those resources.

  • Communications and Relationships
  • Actively develop and maintain effective communication with patients / clients / service users, their families and / or carers
  • Communicate with individuals, carers and other visitors in a courteous and helpful manner, whilst being mindful that there may be barriers to communication / understanding
  • Involve the patient in shared decision-making and obtain their consent before undertaking any care
  • Maintain effective communication with the multidisciplinary team, escalating concerns appropriately
  • Report information to appropriate registered care professional(s) in a timely, sensitive manner,
  • Maintain accurate, clear, concise and contemporaneous records as per Trust policy, communicating and escalated information to the Team as required
  • Ensure ability to manage electronic records system and recognition of the need to protect access (ID, passwords, smartcard and mobile working)
  • Report any accidents or incidents and raise / report any concerns as per Trust policy
  • Contribute to team success and challenge others appropriately
  • Attend and participate in meetings, training and audit as required and share the learning in order to enhance best practice
