Job overview

We are recruiting to a  team of dedicated community nursing staff to work with the Ageing Well and Frailty Service and our Primary Care Networks.  This exciting opportunity means that you’ll be working as part of a combined team of community care staff and PCN colleagues to proactively support people with moderate and severe frailty as part of WCHC’s Population Health Management and NHS Proactive Care model.

You will be based within the Ageing Well and Frailty Service service  working 22.5 hours per week over 3 days.

This is a permanent post

Main duties of the job

This post requires good communication and interpersonal skills and the ability to be adaptable, reliable and approachable.

You will be working with the patients, their family/carers and the wider team (including PCN colleagues) to create outcome goals for the patient to achieve during their time actively within the service, and lead on the development, implementation and monitoring of the patient specific care plan to ensure these goals are achieved. You will be working in both lone capacity and in conjunction with other team members, ensuring clinical standards are maintained using evidence based practice.  You will have a good knowledge of the components of Clinical Governance, Best Practice, Risk Management, Clinical Audit and relevant Professional Codes.  You will be able to evidence excellent clinical skills.
