Do you want to be involved in an exciting new Frailty service in North Cumbria – if so we want to hear from you!

North Cumbria has an ageing population and is seeing more older people living with frailty in our emergency departments and on our wards. We know that a hospital stay makes it less likely that an elderly person can return home and live independently so we have introduced a new Frailty Hub to provide a same day service.

After a successful pilot, the service is now being permanently funded in both Whitehaven and Carlisle; therefore we are looking for dynamic and motivated team members to join our ambitious, multi-disciplinary team. We want to hear from people who are passionate about our vision to provide more care closer to home and enhance the care we provide to older people living with frailty. The NHS 10 year plan is making three big shifts in how the NHS works:

  • From hospital to community – more care will be available on people’s doorsteps and in their homes
  • From sickness to prevention: we’ll reach patients earlier and make the healthy choice the easy choice

We are making steps towards these shifts but we now need to make further moves to help us provide the right care, in the right place, at the right time.

Main duties of the job

Our Senior decision makers will be a composite workforce of those with expertise in Frailty to support expert decision making and management plans.  They will support and supervise a junior team in the assessment, diagnosis and treatment of undifferentiated patients living with frailty.  They will spend time in the Frailty Hub, Community Hospitals and in time in reach to Urgent Community Response.  The role will require close working with the Emergency Department, SDEC, Acute Medical and Virtual Ward teams.  Close liaison with back of house teams to provide seamless transfer of patients when appropriate.  The role will involve senior decisions for the Virtual ward and in time an out of hours commitment to support weekend and evening decisions.

The Consultant Nurse/AHP for Frailty Same Day Services will provide highly specialised clinical expertise, consultancy, expert advice and leadership to patients, carers and Trust staff with a particular focus on people living with frailty and the development of services for this population.

The post holder will fulfil a clear strategic clinical leadership role to promote and improve the care and services to older people across the Trust.
