6 months fixed term post. This is a new specialist Physical Health Team developing within Adult Community Mental Health Services in Black Country Healthcare NHS Foundation Trust
The Trust has undergone a Community Transformation Programme to improve the physical health outcomes for our patients with direction and standards issued by NHS England and NICE guidelines.
You will undertake cardio-metabolic monitoring in Out-patient Clinics and complete occasional home visits.
The role of the Band 6 Clozapine & Physical Health Nurse is to facilitate clinics within Out-patients, complete occasional home visits to undertake annual physical health checks for patients and monitoring of Clozapine, Lithium, or High-dose Antipsychotics. You will monitor physiological measurements, complete ECG’s and take bloods (training will be provided).
You will lead a one-stop Clozapine Clinic, ensuring correct monitoring for all Clozapine patients; blood analysing, physical observations and side effect monitoring. CPMS training will be provided.
We are seeking skilled, reflective and experienced clinician (RMN or RGN) who are able to form good working relationships and will further embed best practice, ensuring parity of esteem to adults with SMI.
You will be flexible, creative and adaptable, will be committed to working in partnership with patients and their carers, working alongside the multi-disciplinary team. Applicants must have excellent communication skills, computer skills, time management and organisations skills are essential. A genuine interest in improving the physical health for patients and should be able to form good working relationships with the multi-disciplinary team to deliver a safe, high quality and effective service. You will need to use your clinical knowledge and skills to prioritise care, act and refer accordingly. You will be expected to provide support and supervision to other members of the team.
