Combined Community and Acute Care group are looking to recruit motivated and enthusiastic Nurse Advisors to work on on a part time or full time basis.

This role is varied ensuring robust procedures are in place to deliver high standards of clinical care and safety.

To be involved in the delivery of current and new projects city-wide,  offering appropriate timely intervention to support recovery and  independence.

This role is fundamental offering a wide choice of referral routes to prevent avoidable hospital admissions, reduce duplication for service users ensuring value for money. The post will involve working with a range of colleagues in primary and secondary health, social care and the voluntary sector.

Applicants must be a Registered Nurse and will have relevant post-registration experience in primary care and be able to evidence knowledge of specialist long term condition management. Have excellent interpersonal and communication skills plus the ability to critically analyse and prioritise workload.  A flexible approach to this role is required as SPA will be developing into a 24 hour service, the post holder will be required to work a variety of shifts including weekends and Bank Holidays.

SPA will provide a comprehensive induction programme which will support you in delivering timely and clinically effective advice and assessments.

Main duties of the job

The nurse advisor will contribute to the delivery of high-quality nursing care through receiving and responding to callers accessing the SPA line.

They will assess health care needs and ensure referral to the appropriate health care provider in accordance with clinical need, and signpost to social and voluntary sector.

The nurse advisor will take responsibility for the delivery of the service during a defined period on duty

To work within Primary Care and interface services to provide a high quality evidenced based support and advice to patients, carers, families and staff across the interface pathway.

Work as an autonomous practitioner and provide a holistic approach to provide a high standard of clinical expertise in the  process of telephone triage assessment, planning treatment education and advice given to patients, carers and other health and social care professionals. To have ongoing responsibilities for the implementation, evaluation and reassessment of patient’s needs.

This role supports the team leader by supervising and leading the staff with in the single point of access in their absence. Promoting positive staff behaviour and facilitating the delivery of treatment to individuals, groups and communities.

To maintain PROUD values and behaviours within the service delivery.
