Our CYPS Early Help Team in Northeast Hampshire (covering Hart & Rushmoor) is a highly supportive, friendly, and innovative service. Guided by the i-THRIVE framework, we focus on providing Getting More Help and Risk Support to children and young people. Asa screening and triaging clinician, you will play a pivotal role in ensuring children and young people are seen by the most appropriate service in a timely way.
What we're looking for:
- Professional registration with NMC, HCPC or Social Work England
- Two year’s post qualification experience of working in the community with children and young people with emotional wellbeing and/or mental health difficulties and their families.
- Experience and confidence in undertaking: mental health assessments, risk assessment, implementing risk management plans and embedding outcome measures.
Main duties of the job
We are seeking an experienced Clinician to provide timely support to CYP within our CYPS Community Team in Northeast Hampshire. This is a dynamic and varied role, offering the opportunity to make a positive impact on the lives of children and young people across the locality
.Key responsibilities include:
- To work autonomously undertaking clinical screening and where appropriate, assessment, of children and young people who may have a range of needs.
- To work alongside medical, nursing and other professional colleagues to ensure that the most effective care and treatment is provided resulting in positive outcomes for that child/young person.
- work within a highly experienced multi-disciplinary team involved in the assessment and treatment of children and young people with a range of mental health difficulties.
- To be able to actively formulate a robust My Safety Plan with young people and their families inclusive of risk assessment and management and positive risk taking.

Main responsibilities
- To provide timely screening and triage of children and young people where the Hub has received a request for support for their emotional wellbeing/mental health needs, including offering discovery conversations where ensuring the safety of the child/young person may be a priority.
- To work individually and jointly to support children and young people be able to access the most appropriate care for their needs.
- To confidently signpost using the iThrive framework, considering where needs may not be based in mental health and where support can be sought from a range of Northeast Hampshire systems partners.
Qualifications required
Degree level qualification in Nursing, Social Work or Occupational Therapy, with associated professional UK registration.
Experience required
Two year’s post qualification experience of working in the community with children and young people with emotional wellbeing and/or mental health difficulties and their families. Experience and confidence in undertaking: mental health assessments, risk assessment, implementing risk management plans and embedding outcome measures.
