Our Crisis Resolution Home Team is looking to recruit a Mental Health Practitioner.

The team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in Telford & Wrekin. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week on a varying rostered shift pattern. We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital.

We are a team that consists of Band 5 and 6 Nurses, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists Social Workers, Administrators and Business Support Staff. As we work with people with all types of mental health presentations, we need to have a range of skills and interests that create a strong and responsive team.

In the role you will deliver a high-quality clinical service to patients and their families.

You will deliver therapeutic interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties.

Main duties of the job

Mental Health Practitioner – Nurse, OT, Social Workers

Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;

  • Provide specialist assessment and care planning in conjunction with service users and carers
  • Evaluate treatment plans and undertake home treatment
  • Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
  • Co-ordinate the work of the Team
