Job summary

Main duties of the job

To work with the GP practices in a Mid & North PCN to support and deliver therapeutic interventions to patients in primary care who do not meet the criteria for secondary mental health services and help deliver the transformation work around "No Wrong Door"

To support the planning and coordination of service users' mental health care in primary care, liaising with teams and agencies as required. To work on signposting to appropriate agencies for ongoing support.

To liaise effectively between primary and secondary care so that referrals can move seamlessly between services, prioritising the patients journey.

To be able to deliver therapeutic interventions to service users whose immediate needs may need help in a primary care setting.

To take an active role in the developments of the Enhanced Primary Care Mental Health Team and with support deputise for the team lead in their absence.
