Job summary

Main duties of the job

You will be required to conduct initial assessments, completing necessary assessment documentation including risk assessments, participating at the weekly MDTs and be part of formulating treatment planning which includes short term interventions or signposting to other services.

In addition to conducting assessments, you will hold a small caseload for individuals receiving short term interventions from the team and will be responsible for conducting the reviews of treatment and ensuring timely discharges. On a rota basis, you will be the designated duty worker triaging referrals and answering calls and queries coming into the team.

We are a specialist NHS Foundation Trust that provides community, mental health and learning disability services for the people of Barnsley, Calderdale, Kirklees and Wakefield. We also provide low and medium secure services and are the lead for the west Yorkshire secure provider collaborative.
