We are recruiting experienced Band 6 mental health practitioners (RMN, OT, Social workers) into our Crisis and Intensive Home-Based Treatment Team in Calderdale and Kirklees.

The successful applicant will have experience of working in mental health, be able to work autonomously whilst knowing the limitations of the role and will have exemplary record-keeping.

We welcome applications from professional backgrounds including mental health nursing, social work and Occupational therapy. The role requires experience and knowledge around coordinating care for complex cases, recovery-based therapy principles, care planning and risk assessment.

This role is outreach work in the community and so a driving licence will be essential.

Main duties of the job

The post holder will work shifts between the hours of 9am and 5pm, Monday to Friday, under the supervision of a CAMHS crisis/HBTT Clinicians and will have Clinical Supervision provided by an experienced clinician.

The primary role is supporting children, young people and their families, who are open to CAMHS and require intensive support in the community to reach their full potential.

The role also involves liaising with other professionals, handing over, assessing young people including care planning and risk assessment.

Using Goal Based Outcomes to monitor young peoples journey through CAMHS with in a discharge-focused, collaborative approach, is vital to this role.

The post holder will use therapeutic engagement skills to inspire young people to maintain their life skills, education and physical activity to aid recovery to ensure children and young people continue to be integrated in the community.

Forming and maintaining professional relationships is a key part of the role.
