Permanent, full-time Band 6 Mental Health Practitioner in Primary Care

We have a great opportunity for a registered Mental Health Nurse to work in our Emotional Wellbeing Service in East Sussex.

This Service is  a joint funded partnership between Primary Care Networks and Sussex Partnership NHS Foundation Trust to deliver specialist mental health support for patients with stable Severe Mental illness. The post is part of the new Neighbourhood Mental Health Team model.

The post will be split across two PCNs providing 50-50 input to both surgeries and so will entail travelling between two sites.

Main duties of the job

The Mental Health Practitioner will be embedded in the GP Primary Care Network (PCN) and be a key member of the Mental Health Multidisciplinary Team (MDT) working with individuals registered with GP practices within a specific PCN with mild to moderate and stable severe mental health needs. The postholder will provide assessment, signposting and brief treatment (1:1 and / or group based) to individuals referred to the service by a number of routes including self-referral PCN clinicians, other members of the MH MDT(including Voluntary & Community Sector (VCS) colleagues, and other primary and secondary care mental health services. - They provide support to Primary Care staff in the local management of patients with common mental health problems. - They ensure that people referred for intervention by the MHP are quickly and efficiently able to access the most appropriate treatment for their mental health difficulty
