Job overview

An exciting opportunity exists for a qualified or newly qualified registered nurse to join the Crisis Centre Lister. We are looking to appoint an enthusiastic and innovative person to work within the team, an integral part of the Crisis Resolution & Home Treatment (CRHTT) service in the North and East of Hertfordshire.

The Crisis Centre supports service users experiencing an episode of acute mental ill health, to help prevent a crisis from deteriorating, offering practical support, providing therapeutic interventions as part of the treatment plan for individuals using the local CRHTTs. Working within the multi-disciplinary CRHTTs, the aim of the service is to provide home treatment as an alternative to inpatient admission. The team embraces a person-centred approach, and a recovery based philosophy. Promoting resilience & crisis prevention, focusing on the whole person in the context in which they live.

Main duties of the job

This position is within the acute care pathway, Unplanned Care directorate. Providing a service for people presenting in acute mental health crisis. The postholder will take an active part in the assessment, planning, implementation and evaluation of care for service users in collaboration with the North, East and South-East multi-disciplinary Crisis Resolution & Home Treatment service (CRHTT).

The post holder will also have a responsibility to support the team in the development and implementation of a multi-professional service within the CRHTT Crisis Centre within the Lister Hospital Mental Health Unit. Providing high quality intensive treatment and support to meet the needs of people in acute phases of mental illness, and their carers’, as an alternative to inpatient admission.

Detailed job description and main responsibilities

Clinical Practice

The postholder will;

Function as a key worker to an individual or group of service users, taking responsibility for the formulation of appropriate care plans.

Be actively involved in the development and facilitation of group activities.

Liaise with co-workers ensuring they are fully conversant with all aspects of service users’ care and treatment.

Identify individual care needs of crisis service users. Plan, implement and evaluate all care programmes working within the local areas CRHTTs MDT.

Maintain close liaison with all departments and staff involved in a service user’s care. Reporting on progress and effectiveness to the appropriate MDT.

Exercise and develop a good working relationship with colleagues in the MDT.

Participate in clinical reviews, case conferences, etc. Prepare reports as required.

Be aware of all policy documents and legal requirements concerning the centre and ensure that these are adhered to, eg local policy, procedures and statutory legislation.

Demonstrate clinical procedures to appropriate grades of staff.

Be fully conversant with and ensuring compliance with NMC codes of conduct and practice.

Management

May be required to act up in the absence of band 6 nurse, but only if preceptorship has been completed. When in charge of the centre, the postholder will ensure the continuity of all care programmes, policies and procedures.

Attend all mandatory training courses as required; fire training, Respect, Moving and Handling, etc.

Attend training courses as agreed within your personal development plan and be responsible for meeting the requirements of your professional registration.

Be aware of, and adhere to all policies, procedures and guidelines pertinent to the centre and the wider Trust.

Education

Assist in the setting of objectives for student nurses on placement at the centre, monitoring and evaluating same.

Participate in all teaching activities for both qualified and unqualified members of staff.

Assume individual responsibility for maintaining current awareness of developments and trends in Mental Health Nursing.

The post holder is required to maintain registration with the NMC.
