Case Management of specialised commissioned within the Provider Collaborative within the defined population: CAMHS Adult Secure Services Adult Eating Disorders Perinatal
Specific Responsibility for the Provider Collaborative population but will support fellow Case Mangers in their roles within the Provider Collaborative (PC), Regional and National role.
The role is integral in ensuring that Cumbria, Northumberland, Tyne and Wear NHSFT and Tees, Esk and Wear Valleys NHSFT as lead providers meet their accountabilities and responsibilities in terms of commissioning for quality. To provide clinical assurance to the PC.
As a Case Manager, the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for all service users from their Provider Collaborative (PC) in specialised mental health care settings, within the NHS and Independent sectors.
Provide clinical assurance and quality oversight for the defined population of commissioned services.
Managing individual patient pathways to ensure quality services and care planning outcomes are achieved.
Work and engage with internal and external stakeholders on a range of sensitive issues. Nurture key relationships and maintain networks internally and externally, including national networks.
To provide links with Integrated Care Systems (ICS) and their commissioned services within the PC to ensure there is a co-ordinated approach to care and discharge planning, which results in robust and effective care packages.
Detailed knowledge of legal issues pertaining to the specialist service, Detailed operational knowledge of specialised secure, forensic and CAMHs, Deaf, Eating Disorder and Perinatal services, including risk assessment/management. Knowledge of the CAMHS, Forensic, Deaf, Eating Disorder and Perinatal and secure agenda/policy and its application in specialised services.
Able to perform to high level in clinical setting, holding services to account and ensuring effective use of Provider Collaborative resources. Case management skills, focussing on the needs of the individual and the context of their pathway in relation to all clinical and legal processes.
The post holder will be able to travel across the PC region as well as national travel requirements on a regular basis to support the quality oversight of services and patient care.
