To have a responsibility for the co-ordination and development of the memory clinics. To have a key role in the on-going assessment, treatment, and support to people with dementia, their carers, and relatives. To collaborate with members of the integrated older peoples’ community mental health teams, to provide professional advice and support where required. To promote and develop evidence-based practice working within NICE guidelines and MSNAP Standards.
To accept care co-ordinator responsibility for a clinical caseload.
To initiate assessments and treatment programmes in service user’s homes and at the memory clinics.
To facilitate the inclusion of social care services in the care plan in line with assessed need, agreed eligibility criteria and agreed procedures.
Respond to the needs of people in an honest, non judgemental, and open manner, which respects the rights of individuals and groups.
Actively engage with service users and their carers in the provision of holistic, needs-led care, which takes account of the physical, psychological, emotional, social, and spiritual needs of individuals.
Have an understanding of the needs and rights of carers and undertake Carers Assessments with a view to offering supportive services.
To maintain accurate RIO records, including core and risk assessments, care plans, progress notes and contacts as required.
Key Tasks and Responsibilities
To hold professional responsibility for assessing individual care needs, risk assessments, development, implementation, and evaluation of care plans, in line with OXLEAS Foundation Trust CPA policy. With particular focus on service users with Dementia.

To demonstrate understanding and use of clinical supervision to reflect upon and modify own and others practice.
To ensure that own practice remains within legal, ethical, and professional parameter and is open to scrutiny from peers.
Most hold a current, valid first level registration with the Nurses and Midwifes Council.

Have a good understanding of the Mental Health Act 1983 and the Mental Capacity Act 2007. Undertake mental capacity assessments and have an understanding of their responsibilities under both Acts.
Have an understanding and knowledge of the Care Act 2014 and its implications to practise.
Demonstrate an understanding of Safeguarding Adults policy and procedures.
Ensure that service users and their carers are up to date in their knowledge including having a copy of their care plan, information about treatment and the role of the care co-ordinator.
Assist patients with social and practical activities, such as education, leisure and financial management and assisting in developing and maintaining personal and social networks with families, carers, and others in line with the social inclusion model of care.
Advise and assist patients in maintaining daily living skills such as personal hygiene, self-care skills, social skills, and nutritional management.
Implement evidence-based practice in all aspects of clinical care, developing an ongoing awareness of significant nursing and other mental health research relevant to the care group.
Develop effective working relationships with patients, families and carers by facilitating engagement and therapeutic co-operation through the use of flexible and responsive strategies.
Facilitate groups within the community for clients, carers, and relatives.
To provide advice, education and work in co-partnership with residential/nursing homes.
Be responsible for the administration, carriage and storage of medication in accordance with relevant policies and clinical procedures. To maintain drugs records as agreed policy.
To act as a resource within the CMHT, giving advice and offering assistance as appropriate [e.g. joint visits]

Research
Participate in any research, audit, Clinical Governance or Clinical Effectiveness projects, as required.

To communicate effectively within the multidisciplinary team and with service users and carers on a range of issues that are often complex in their nature and in complex situations.
Participate in supervision and Professional Development Reviews and demonstrate reflective practice through the Knowledge and Skills Framework.
To maintain electronic and written clinical records ensuring confidentiality is always maintained.
Provide supervision, mentorship and leadership for junior staff within the Memory Service & Student Nurses, appropriate.
