Job overview

The Macmillan Lung Cancer Support Worker plays a key role in supporting patients and their carers to navigate the complexities of the health and social care system during and after cancer treatment. Working collaboratively with the Lung Clinical Nurse Specialist (CNS) team and the Macmillan Cancer Navigator Service, the post holder ensures patients have a single point of access to services, including rapid re-entry for those with urgent or specialist needs.

The role focuses on coordinating care for patients with non-complex needs, in line with the National Cancer Survivorship Initiative risk stratification model (level one – supported self-management), as well as coordinating education and support to promote independence and wellbeing. The post holder provides clear information and guidance, delivering personalised care that empowers patients and enhances their experience throughout the cancer pathway.

Main duties of the job

  • Contribute to the holistic needs assessment and the development of an individual care plan under the direction of the CNS
  • Coordinate care by collaborating with the Macmillan Navigator service who provide a single point of access, including rapid re-entry into the system for those people identified as  having urgent or specialist needs.
  • Make pre planned outgoing telephone calls to patients to assess ongoing needs and monitor progress, liaising with the speciality CNS and other members of the healthcare team as  appropriate in order to initiate a proactive prevention approach.
  • To undertake any other such duties as may be required from time to time which are  consistent with the responsibilities of the grade.
  • To participate and display a willingness to learn, develop a relevant technical competency / area of interest and keepup to date with current national and local issues and developments.
  • To be responsible for informing line manager of any issues relating to the ability to safely carry out duties.
  • To ensure confidentiality at all times in accordance with Trust policy and procedure.
