Job overview

This role will support the coordination of care for Lung cancer patients throughout their diagnostic and treatment pathway. There will be a focus on ensuring the provision of good information to patients and their families and being a point of contact for them in the case of any general queries.  The role requires detailed understanding of personalised care, supports Lung CNS Team to undertake Holistic Needs Assessments and other elements of the Recovery Package. The role will work to improve the overall experience of patients with lung cancer and their carers. There are independent working elements involved with in this post (under supervision) which requires a degree of initiatives and decision-making ability.

Main duties of the job

Provide patients with lung cancer and their carers with appropriate information on general supports, guide them for wellbeing supports and benefits supports to local and national organisations. Complete holistic needs assessment (HNA) for patients at diagnosis and various points during and after their treatment. Provide regular proactive contact with the patients and carers to identify and act on the changes in need. Monthly data report on the HNA completion. Keep electronic record up-to-date with accurate documentation.  The support Lung CNS team with triaging telephone calls and emails, provide patients with appropriate initial guidance and keep the Lung CNS team informed. Discuss concerns with the LCNS team in a timely manner to provide safe care
