Job overview

This a fixed term post comprised of 8 Direct Clinical Activities and 2 Supporting Professional Activities providing outpatient Hepatology, inpatient Gastroenterology and Hepatology and Endoscopy services. The post-holder will be on call for acute gastrointestinal bleeding rota currently covering both THH and Harefield Hospital on a 1 week in 7-8 basis, dependent upon recruitment. This attracts an additional 1 PA. The intention would be to remove the Harefield commitment from the department in the future, with a consequent 0.5PA reduction. There is an availability uplift for on-call from THHFT (5%). There is no General Medical on call commitment for this post. Applicants must hold a CCT in Gastroenterology and GIM or be within 6 months of CCT to be eligible to apply for the post.

Main duties of the job

This is an additional post to support the development, resilience and sustainability of our Hepatology service. The successful candidate will join the existing Gastroenterology and Hepatology consultants, Dr Grover (Gastroenterology and Hepatology and Clinical Lead), Dr Lean (Upper GI Cancer Lead), Dr Postgate (ERCP, IBD and Interventional Endoscopy), Dr Rajendran (Endoscopy Training lead, IBD and General Gastroenterology), Dr Rameshshanker (Endoscopy Lead, ERCP, IBD and General Gastroenterology), Dr Oke (Nutrition Lead, IBD and General Gastroenterology). The department works as a cohesive clinical team with complimentary interests.  We are fortunate to have a bespoke, co-located Endoscopy Unit and offices, alongside the specialist nursing team.

Duties/ responsibilities of the post

We are looking for an enthusiastic Hepatologist, Gastroenterologist and General Physician to join our busy DGH Hepatology service.  Currently the vast majority of direct clinical reviews are performed by non-consultant grade members of the Team (CNS x2, PA, Band 4). The CNS's have presented our service delivery model at multiple EASL conferences and received widespread acknowledgement and praise for the streamlined model.

Detailed job description and main responsibilities

Applicants must have full GMC registration as well as experience of working in the NHS. They should have or be within six months of CCT in Gastroenterology and Medicine.

The post holders will, together with their consultant colleagues and other health care professionals in the Division, provide an effective and comprehensive service for our patients and support continuing service developments. The Gastroenterology and Hepatology Clinical Lead will be the immediate line manager.

The post holders will be able to participate in the on-call GI bleed/ Gastroenterology rota on an approximately 1:8 basis, with full recruitment.

The primary focus of this post is to support the Hepatology service.

The post-holder will work closely with their colleagues, be responsible for providing a high-quality Hepatology and Gastroenterology service to the residents of Hillingdon, and for development of services appropriate to these ends. This includes vetting of Hepatology referrals and review of these patients in clinics (face to face, telephone and paper/ virtually).

The post-holder will participate in the ‘bleeding rota’; providing emergency out-of-hours Gastroenterology cover to both Harefield Hospital and Hillingdon Hospital. Currently this is non-resident, with future job planning this may involve a regular on-site commitment. We are negotiating to withdraw from the Harefield commitment, to focus clinical time at the Hillingdon site.

The post-holder will, within a Team, be jointly responsible for running an inpatient and outpatient service for Hepatology and general gastroenterology including an advisory service for colleagues.

Attend the relevant MDT meetings including: Gastro Radiology, Upper GI cancer MDT, West London Histopathology and other pan-NWL/NCL meetings as appropriate.

The post-holder will participate with his/her colleagues in the training and professional development of medical and other professional staff for whom he/she is responsible.  He/she will also be involved in the postgraduate and undergraduate teaching programme and General Practitioner education.

The post-holder will be expected to continue his/her own professional development (CPD) and revalidation in line with guidance from the relevant professional bodies and within the framework of clinical governance. The post-holder will be expected to help the department meet its objective of JAG accreditation.

The post-holder will be expected to work with the other Gastroenterology and Hepatology Consultants to ensure the Trust meets the various national waiting time targets.

The post-holder will be expected to provide clear evidence of their SPA activities. This includes clinical and educational supervision, audit and personal CPD. Personal CPD must be included as part of annual appraisal which is mandatory and required for the revalidation process. Performing / leading at least 1 medical audit per year.

The post-holder will be encouraged to develop any areas of expertise of special interest that he or she may have in Hepatology, in accordance with the service needs of the Department.

Participation in the medical advisory and liaison work of the hospital Consultant Staff Committee and to share the workload of the involvement in any other committees relating to the Medicine and Gastroenterology Departments, for example (not an exhaustive list) Clinical Governance, Safeguarding Adults, Transfusion, Resuscitation, Medicines Management etc.

To collaborate with the Lead Clinician and the Senior Nurse Management Team in the development and implementation of all relevant policies for the running of the department.

To work with the consultant team in the recruitment and appointment of junior medical staff in consultation with the Department of People and Development and the Unplanned Care Senior Nurse Management Team. They will help to organise and give guidance to the deployment of junior staff duties including allocation and supervision of clinical work where necessary and support their use of the Trust Digital systems.

The post has been created due to the increased workload generated by Liver disease detection programmes such as the Emergency Department Blood Borne Virus programme and RM Partners Hepatocellular carcinoma surveillance/ early detection programme.

Any interested candidates are strongly encouraged to visit the department and contact the Departmental Lead, Dr Bob Grover ([email protected]). We hope to advertise a substantive Hepatology post within 6-9months.

**Agencies should contact the Trusts Temporary Staffing Team rather then the service directly.**
