The geriatric medicine team at SaSH is expanding and the future is exciting. We are part of the National Neighbourhood Health Implementation Programme (NNHIP) trial site and part of Surrey East which hosts one of the national Frailty collaborative hubs.
We are looking for a colleague to join our wonderful team. This is a fixed term post to help us develop our community geriatrics in reach into East Surrey. The post will be working 4 days a week in the community, integrated with the urgent community response, virtual ward and intermediate care teams. It will be delivering admission avoidance and proactive case management in older people's own homes. One day a week the post will be working within the acute trust on the frailty unit.
This post would suit someone with prior community medicine experience and a passion for the development of such a service. The post will work with the First Community Healthcare team (community trust) and SaSH (acute trust) in partnership.
