Main duties of the job
In this vital role, you’ll coordinate SJR panel meetings, manage training, and maintain accurate records to support our Learning from Deaths programme. You’ll work closely with clinical teams, handle sensitive information, and use systems like InPhase and Microsoft Office daily. We’re looking for someone with strong IT skills, excellent communication, attention to detail, and the ability to work both independently and as part of a supportive team. You’ll make key decisions such as:
- Prioritizing and scheduling meetings and training to meet service needs. • Deciding when to escalate cases for further review. • Determining the best way to communicate findings and actions to ensure lessons are shared and improvements are made.
Detailed job description and main responsibilities
- Work with the Associate Medical Director in Quality and Safety and the Patient Safety team to administrate and coordinate the activities required to support, develop and oversee the Trust’s Learning from Deaths programme and the quality improvement priorities which come out of it.
- Coordinate and administrate the Learning from Deaths Committee.
- Manage the Learning from Deaths email inbox for queries relating to the Trust’s Learning from Deaths Programme which include Morbidity & Mortality Reviews, the Medical Examiner’s Service process, SJR processes and training.
- Action the manual escalation of SJR where this is required for example where cases are referred to the SJR process as a consequence of a Coroner’s regulation 28, Complaints team and PALS.
- Ensure that all cases referred to the SJR process are reported via DATIX where appropriate.
- Coordinate and administrate SJR cohort reviews when required to investigate mortality alerts or specific concerns about an area of care.
- Coordinate and administrate the SJR scrutiny panel including the following tasks:
Identify cases for the panel review
Generate an attendance list of the panel review and contact relevant leads/SJR reviewer with video conferencing or venue information as appropriate
Monitor the receipt of the SJR prior to panel and attach to Datix where appropriate
For all vulnerable patients being reviewed at the SJR scrutiny panel, liaise with the relevant specialist team (LD/Mental Health Liaison/ Drug and alcohol support) to ensure completion of the LD/ mental health/homeless tri-morbidity template prior to the panel review
Ensure healthcare records for the case being presented are available for the panel
Take notes of the discussion held and record actions/lessons learned, record the panel decision on whether there were lapses in care and if the death required escalation to the Serious Incident Group
Where appropriate, update DATIX following the panel review to include identified actions and lessons learnt
Where appropriate, liaise with the Risk and Patient Safety team to organise a Quality Issue Report where care delivery problems are identified that relate to care from providers external to the Trust
Assist in preparing a summary report on the cases reviewed by the panel for the Learning from Deaths Committee to include identified actions and lessons learnt
