The Adult Learning Disability Service is excited to bring this opportunity for one experienced and ambitious practitioner (Nurse or AHP) to join our team as a Community Specialist Health Transition and Referral Coordinator.
This pivotal role focuses on triaging and screening all referrals for young people with a learning disability aged 17 ½ to 18 into the adult learning disability team in South Essex.
Working across health, education, and social care, you'll collaborate with mainstream services by developing pathways and providing education and training to other health services so that, where possible, they are able to make reasonable adjustments to be more accessible to this service user group.
The role is community based and the post holder will be expected to visit young people in their homes, school and/or day opportunities to support the completion of the triaging process. Home visits will be across areas from Southend, Castlepoint and Rochford, Basildon, Brentwood, Wickford and Thurrock. So a car driver will be essential.
The post holder will also support the movement of service users between the wider MDT using the internal referral pathway and lead weekly meetings with the wider MDT preparing information for internal referrals, and complex case discussion.
Main duties of the job
- work in close collaboration with other professionals to provide a comprehensive assessment and screening service to young people with learning disabilities entering transition (17 ½ to 18). The transition worker, with the wider MDT will develop care plans that reflect the needs and wishes of the service user and their family/carers; agreeing safe clinical outcomes which address the needs of the service users and meets individual health needs. To collate accurate and relevant health and social care information in order to provide advice, guidance to the young person as they approach their 18th birthday.

Detailed job description and main responsibilities
- The post holder will have the clinical skills and qualification relevant to the delivery of care in the service area.
- The post holder will comply with and promote the Code of Professional Conduct as stipulated by their relevant professional body.
- The post holder will provide a holistic approach to service user assessment, treatment, review and management of packages of care. Utilising resources effectively within the service area.
- The post holder will undertake a range of comprehensive and evidence based clinical risk assessments and associated care plans that are relevant to the service users care needs.
- The post holder will review and communicate service users risk assessments and management care plans in accordance with trust policy ensuring meaningful service user, carer and MDT involvement throughout the process.
