An exciting opportunity has arisen for a motivated, compassionate and enthusiastic Band 5 Nurse to join the Learning Disability Intensive Support Team. You will be joining a highly supportive multidisciplinary team where regular supervision, open communication and professional development are prioritised - ensuring you feel valued, guided and confident in delivering best possible care. You will work across Hull and East Riding with a requirement for a full UK driving license and vehicle.
The successful applicant will join a multi-disciplinary team, providing a holistic approach in the care of those who access our service. As such we are looking for a flexible, forward think nurse who has the service user at the centre of everything they do. Within this position there will be the requirement for rotation onto the Townend Court inpatient unit.
Your experience will be vital in maximising a positive patient journey, primarily preventing admissions in the community however also, should the need arise, supporting the admission, treatment and subsequent discharge. Across both services we work as part of a multi-disciplinary team to provide a holistic approach in the care of those who access our service.
You will work to improve the health and well-being of individuals with learning disabilities by assessing needs, creating care plans, and supporting them to live independently.
You will work with individuals and their families to improve and maintain physical and mental health, overcome barriers to healthcare and social inclusion, provide advice, and support with daily life skills.
This role involves managing your own caseload, coordinating with other professionals, and ensuring the person is at the centre of everything you do.
