**12 Month Fixed Term/Secondment Opportunity**

We have an exciting opportunity for an experienced Learning Disability Nurse to join the Complex Health Hub at NNUH.  This role forms part of the wider Complex Health Hub which is responsible for Safeguarding, Mental Health, Dementia, Learning Disability and Autism, Substance Misuse, MCA & DoLs, Homelessness and Prison Healthcare.

The post holder will provide specialist nursing input to ensure that children and young people with learning disabilities and/or autism receive equitable healthcare access across the range of services offered at NNUH. The post holder will work alongside internal clinical partners (inpatient, elective and outpatient) and external partners (family and advocacy groups, community professionals). The post holder will work closely with children, young people, and their families and carers to ensure that care is adjusted to meet their individual needs. The post holder will lead by example, demonstrating the importance of person-centered care at all points of patients’ healthcare journeys.

Main duties of the job

Lead the delivery of effective, accessible and high-quality care which is responsive to the needs of Children and Young People with learning disabilities.

Provide specialist advice and support relating to Learning Disability and Autism across a range of Children’s and Young People’s services throughout the hospital

Support Children’s and Young People’s services in delivering accessible care to people with learning disabilities and/or autism

Work with Children and Young People, their families and carers across a range of inpatient and outpatient settings to ensure equity of access

Closely collaborate with healthcare and multi-agency professionals and develop and maintain effective links between hospital services, community services, private and voluntary sectors

Detailed job description and main responsibilities

  • Ensure practice is in accordance with the NMC code of Professional conduct and work within limits of competence
  • Ensure the integration of individualised health plans, hospital passports and admission plans into the practice of healthcare professionals within the acute hospital setting
  • Provide specialist advice and support to hospital staff, ensuring there is a clear pathway of care for people with learning disabilities and / or autism
  • Prioritise and respond to referrals made to the service
  • Respond to unplanned/emergency admissions where a need for support/advice is identified
