Job overview

The post holder will provide a lead role in transitions and discharge for young people within the service, they will work closely with other members of the multi-disciplinary team as well as partners across the region. There should be significant focus on the reduction in length of stay. Working as part of the multi-disciplinary team in a senior leadership role, the post holder will work autonomously managing a small individual case load of complex cases.  There will be additional responsibilities such as supervision of other staff, supporting them and providing advice in dealing with complex cases as well as attendance at a variety of meetings.

Main duties of the job

The post holder will maintain comprehensive and up to date written records of assessments, CPA plans, evaluations and on-going process notes in line with Trust Policy and to share these with the MDT and external agencies as appropriate.

  • Ensure the service user and their families are central to and fully involved in the plan of care and have copies of care plans.
  • Complete complex social and statutory reports.
  • Provide a senior role in multi-disciplinary and multi-agency forums
  • The post holder will form part of the senior leadership team contributing to the on-going development of inpatient CYPMHS
  • To be aware of and adhere to all professional and Trust Policies including the reporting of incidents and accidents, complaints procedures and the management of confidential information.

Detailed job description and main responsibilities

The successful applicant will

  • provide a senior role in multi-disciplinary and multi-agency forums
  • Provide advice and guidance to other staff on complex cases
  • work in partnership with Service Users and partner agencies (e.g. housing, Benefits, Social Services etc.) to facilitate benefit claims and other financial and housing support.
  • maintain links with carers and ensure that Nearest Relatives are informed of their rights under the Mental Health Act, 1983
  • ensure that the needs of service users children are understood, making referrals to social care where required and ensuring that all relevant parties are involved in child visiting arrangements.
  • ensure that Statutory Social Circumstances reports are available for mental health tribunals and/or hospital manager’s hearing.
