KCHFT is offering an exciting opportunity for an experienced Pharmacist who has recently worked within multidisciplinary teams to support patients living with frailty. We are looking for a Pharmacist with either hospital or primary care experience who is passionate about improving pharmacy services to support patients to live well at home
You will be responsible for leading the pharmacy frailty enablement teams within the community hospitals and domiciliary settings. These are currently 2 teams of pharmacists, technicians and pharmacy support workers, one within each setting.
You will have responsibility for developing and implementing an integrated clinical pharmacy frailty service within these settings, ensuring that the team work to the same aims irrespective of the setting, with a patient and MDT centred focus.
You will develop collaborative relationships with internal KCHFT clinicians and frailty service leads to understand where the team can add most value.
As a community Trust many of our roles require individuals to be able to drive to perform their duties. As such you will be asked to confirm you have a full UK driving licence where driving is a requirement of the role to be able to proceed with your application.
