An exciting opportunity has arisen to appoint a full time band 5 Keyworker (Pending Agenda for Change job evaluation) to support our established Keyworker Team, based at Dragon Square, Chesterton.

The Learning Disability and Autism Keyworker Team, works with Children and Young People 0-25, with a Diagnosis of a Learning Disability and/or Autism who are on the Dynamic Support Register due to being at risk of Tier 4 Hospital admission or care package breakdown.

The purpose of the Keyworker role is:

  • To understand and promote the needs of the child/ young person and their family.
  • To ensure that local systems are responsive to meeting their needs in a holistic and joined up way
  • To improve outcomes for children and young people with learning disabilities, autism or both.
  • To deliver the following competencies as part of their role

Main duties of the job

Key Duties/Responsibilities

Provide information and support to CYP, their families and carers to ensure that personalisation approaches are embedded in CYP plans and that creative solutions are considered to meet the individual’s needs supporting with personal health budgets and integrated personal budgets.

Support the Lead Keyworkers, and specialist keyworkers in the delivery of the project across North  Staffordshire and Stoke on Trent.

Provide support, information and signposting to CYP, their families and carers.

Act as a single point of regular and consistent contact for a caseload of CYP.

Work with CYP and, if applicable, their family/carer to develop a personalised plan.

Utilise the Dynamic Support Registers (DSR) agreed Blue, Red, Amber, Green rating system to capture relevant information that will be used to support the step-down process in line with the approach within the project model.

Working with CYP, parents, carers and people with lived experience to support the development of the Keyworker Community of Experience.

Ensure a person-centred approach at all times, responsible for developing individualised care plans utilising specialist advice alongside the team around CYP.

Communicate effectively with CYP, taking into consideration their cognitive developmental level, behavioural/emotional state, speech and language difficulties or psychiatric presentation.

Detailed job description and main responsibilities

Plan care on behalf of CYP and their families/carers, e.g. organize MDT’s to bring the professionals together to be able to cco-ordinate care in a holistic manner.

Responsible for managing an ongoing caseload prioritising own workload, balancing the needs of CYP and their families/carers often with competing priorities.

Responsible for developing, planning, and organising activities and a number of ongoing tasks and activities.

Ensure all documentation and data collection is accurately completed in line with systemwide policies and GDPR.

To effectively and appropriately maintain CYP records on systems.

To use office equipment in an appropriate manner, reporting any faults following the recognised organisational procedure.

Exposure to a VDU is frequent in line with H&S guidelines.

Decisions and judgements

To bring together information and facts, analyse, interpret, and make judgements, potentially in difficult, challenging and/or complex situations. To then explore and compare potential range of options understanding the risk, impact, and benefit of each with the individuals involved.

Take time to assess and understand CYP’s communication needs based on the analysis of a range of facts or situations and adapt their approach individually for them. 24. Respectfully challenge blocks to collaborative working and individualised care and contribute to a solution focussed approach.

Exercise judgement, accountability and responsibility using skills, knowledge, and expertise in changing environments, across agency boundaries and in unfamiliar situations on behalf of CYP and their family.

Knowledge of relevant Trust policies and procedures e.g., Fire, Health & Safety, Data Protection/Security of Information.

Contribute to policies, procedures and protocols within the service as required.

Implements policies, procedures, and protocols relevant to own work area, setting standards for procedures within designated service area(s).

Works autonomously, prioritising tasks in accordance with agreed objectives, organisational policy/guidance, and defined parameters.

Work is self-managed rather than supervised.
