Be a registered General Nurse & must be willing to complete the Nurse Prescriber Course
Take responsibility for a defined cohort of patients with complex or higher acuity needs within the District Nursing caseload
Use your extended clinical skills & knowledge to assess, plan, & implement evidence-based treatments that improve health outcomes
Collaborate with the multi-disciplinary team to deliver person-centred care that promotes independence & self-management
Lead by example, supporting colleagues & contributing to the proactive management of patients in the community
Support the District Nurse/Community Matron in managing higher acuity patients & deputise when required
Support staff development through supervision, mentorship, & preceptorship programmes
