Main duties of the job

This is a fantastic opportunity to provide high-quality, patient-centred physiotherapy across a range of community settings. You’ll work autonomously and as part of a dynamic multi-disciplinary team, which includes nurses, occupational therapists, social workers, dietitians, and more.

Your day-to-day will include:

  • Providing expert care in patients’ homes and intermediate care placements
  • Managing your own caseload across varied and often complex conditions
  • Working closely with GPs and wider healthcare professionals to prevent hospital admissions and support early discharges
  • Participate in our flexible weekend and late duty rotas Community ward cover as required

We are looking for:

  • HCPC-registered physiotherapist with relevant NHS or equivalent experience
  • Broad clinical experience including MSK, neuro, respiratory and elderly care
  • Able to work independently and manage risk, including lone working in the community
  • Someone who thrives in a fast-paced but friendly environment
  • A driver with access to a vehicle for community visits
