This is an exciting opportunity to join our dynamic and forward thinking Integrated Placement of Care Hub Team.
The hub supports inpatients who are being discharged into the community adopting a home first approach. The hub assesses, triages and provide discharge routes for a wide variety of patients, either in their own home, in short stay bed based or long term care.
The range of patients we support is vast, from rehab to palliative care both in and out of acute settings. The Band 6 practitioners rotate across services, and are fully supported/trained under the direction of the team leaders and clinical service lead.
To support this level of complex communication, this team are the founders of the HSJ award winning IDT system, a web based, user friendly system which is used by multiple partners and the envy of many trusts. We are continually striving to improve and are excited by the many new projects that lie ahead of us.
We are looking for highly motivated, friendly and professional staff to expand our team and help us to achieve safe, effective patient flow from hospital and out of short term settings.
In addition you should be experienced in complex discharge in either acute or community settings, have a passion for improving patient flow and be confident in engaging a wide range of disciplines in order to achieve the right outcomes for your patients.
You will act as an practitioner providing expert knowledge of discharge planning to other professionals both in the acute & community care settings.
Manage and support the leadership team to reduce service users length of stay and promote patient flow across all settings.
Will have the ability to make autonomous decisions regarding the ongoing care needs of our service users.
Liaise and communicate with the wider multidisciplinary team, patients & family members- advanced communication skills essential for this role.
Deliver specialised assessment.
Complex case management.
We cover a 7 day a week service, therefore any successful candidate would be expected to work a number of weekends and bank holidays.
Travel is needed as part of this role; please demonstrate on your application form how you would meet this requirement.
Show effective communication skills, maintaining confidentiality at all times.
Ensure patients needs are assessed and ongoing care is effectively implemented.
Maintain accurate records and documentation both written & electronic.
Take responsibility of own learning maintaining mandatory training, appraisal & professional registration requirements.
Active participation in improving other professionals understanding & learning regarding patient flow and discharge planning.
Comply with CCICP policies & procedures, including responding to safeguarding concerns appropriately.
Deputise for the leadership team when needed, demonstrating effective clinical leadership, organising other team members.
Provide pastoral support to junior members of the team, including the expanding GNA service.
