Job overview

The Flow Support Worker will play a prominent role in ensuring the delivery of flow within LSCFT Inpatient units. This will be achieved through developing excellent internal Communication pathways, the utilisation of the internal escalation process and ensuring the multi-disciplinary team adhere to the Department’s internal professional Standards.

The post holder will ensure the timely flow of patients through the area to help identify available capacity and assist to place patients appropriately, supporting the Discharge Practitioner and Flow Lead working within agreed procedures and protocols.

Main duties of the job

The post holder will work amongst the Kentmere Multi-Disciplinary Team and Integrated Discharge Team. The post holder will screen patients on admission to understand complex needs that will require further assessment to promote safe, timely, and sustainable discharges for patients on the ward.

The post holder will work with the Multidisciplinary Teams (MDTs), community teams, adult social care, housing providers, patients, and their families to identify support needs and address any barriers to discharge.

The post holder will proactively participate in and contribute to weekly case management meetings providing updates to the team and offering specific advice and guidance to colleagues.

The post holder will be expected to attend ward meetings, discharge planning and patient ward reviews, and professional meetings and to participate professionally, occasionally being required to assertively represent the outcomes of assessments and, where appropriate, challenge the views of other professionals.

The post holder will establish and maintain positive & effective working relationships within the wider multi-organisational team environment both with immediate colleagues and members of other teams.

The post holder will explain complex processes and clinical information to patients and their carers or representatives in a way that they are understood.
