Job overview

As a band 6/7 Hospital at Night Co-Ordinator, you will be required to provide experienced acute care and advice, acting within your professional boundaries. Demonstrate safe, clinical decision making and expert care through triaging the calls/ bleep system, to provide timely intervention for patients by prioritising the clinical workload for the appropriate clinical teams across the Trust. The Hospital at Night Co-Ordinator will be the first point of call to allow the clinical staff to thoroughly assess patients without being distracted by the bleep. You will work collaboratively with the (night time) out of hours multi-disciplinary team on duty to meet the clinical needs of the patient and support the team in the delivery of high-quality care with the aim to prevent unnecessary patient deterioration.

Main duties of the job

  • To work within your responsibility and accountability within this role in line with your professional code, safeguarding and the well-being of patients.
  • To co-ordinate the Hospital at Night handover process as per Trust guidance and operational protocol (H@NT Handbook)
  • To demonstrate clinical leadership skills and evidence of current relevant clinical practice.
  • To liaise with clinical services and senior clinicians to expedite the care delivery to patients highlighted out of hours at night.
  • The post holder will work autonomously, managing the allocation of tasks and clinical needs out of hours.
  • To contribute towards the continuing education for staff and self, acting as an advocate for the patient and their families.
  • Must achieve and maintain specific role related competencies for this role.
  • The H@NT co-ordinator should be caseload free and is not expected to directly review patients, but to ensure timely escalation and clinical review.
