Job overview

The post holder will provide comprehensive, professional and confidential administrative and operational support in the delivery of a number of projects within the Home first Programme. There will be an emphasis of capturing activity from the Home First in reach MDT who currently support the D2RA The post holder will be responsible for managing their own workload and will be given delegated responsibility for supporting certain aspects of projects.

This will include overseeing delegated activities and ensuring that timescales are being met. To develop and maintain information systems that assist in the delivery of projects ensuring that progress and Home First activity is accurately captured and reported.

The post holder will have excellent interpersonal and organizational skills to act as a channel of communication between the Home First integrated MDT, Project Home First Leads, Senior Colleagues in the Health Board, other NHS Organizations, Welsh Government and other external partners

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply

The post holder will provide comprehensive, professional and confidential administrative and operational support in the delivery of a number of projects within the Home First Programme. There will be an emphasis on capturing activity from the Home First in reach multidisciplinary team who currently support the discharge to recover and assess model as set out by Welsh Government.

The post holders will be responsible for managing their own workload and will be given delegated responsibility for supporting certain aspects of projects. This will include ;

  • Overseeing delegated activities and ensuring that timescales are being met.
  • Developing and maintaining information systems that assist in the delivery of projects ensuring that progress and Home First activity is accurately captured and reported
