We are looking for someone with a passion for community rehabilitation who can help the Community Independence Service (CIS) make Hammersmith & Fulham a great place to grow old!
You will be key in delivering advanced holistic assessments and rehabilitation, implementing effective programmes during a patient’s episode of care. You will also be supervising and a mentor for junior staff members, develop and maintain relationships that integrate health and social care to support and reduce the need for hospital admission for patients, ensuring that they maintain their ability to engage in daily activities. The post holder will be an experienced and skilled Allied Health Professional who will be able to work across the Community Independence Service (CIS) but based primarily in community rehabilitation team. As a senior member of the CIS Team, you will be expected to be a clinician with a broad range of competencies who is able to adapt to a wide variety of clinical circumstances. You will be part of a community team where your ability to make clinical decisions will aid client recovery, maximise functional independence and avoid unnecessary hospital admission. You will undertake comprehensive assessments and provide complex interventions that will have an impact on clinical decisions. You will be expected to work autonomously whilst drawing assistance from other colleagues in primary and secondary care. Your excellent caseload management and communication skills will enable you to prioritise client care and manage our own caseload. You will also work closely with senior clinicians, practitioners, social workers and other health or social care professionals to establish and maintain a whole systems approach for our clients. The post holder will provide community based rehabilitation to clients who are referred with varied and complex needs ensuring holistic assessment and community care pathway planning is undertaken to maximise the rehab potential for the client
