We are seeking a Highly Specialist Occupational Therapist (OT) to join our dynamic Multi-Disciplinary Team (MDT) in providing a seamless, patient-centred service.

In this role, you will develop, implement, and evaluate the Merton Enhanced Home-Based Rehabilitation Team (MEHBRT), ensuring that patients receive timely, high-quality care that supports their independence and wellbeing.

You will assess referrals from hospitals for patients ready for discharge who continue to have health or social care needs. As a clinical leader, you will provide guidance and support to occupational therapists, junior staff, rehabilitation support workers and MDT students, fostering a culture of learning, collaboration and excellence.

This role involves managing your own complex caseload, contributing to service quality and performance, and building strong, inclusive partnerships with statutory, voluntary and independent organisations. You will deliver compassionate, competent care to patients with physical and cognitive difficulties in both community and care home settings.

Main duties of the job

  • Function as a vital member of the MDT, ensuring seamless, patient-centred care.
  • Develop, implement and evaluate the MEHBRT, ensuring timely and high-quality care.
  • Clinically assess referrals from hospitals for patients ready for discharge who require ongoing support.
  • Provide local clinical leadership and guidance to occupational therapists and the wider MDT within the CSU.
  • Educate, supervise and support junior occupational therapy staff, rehabilitation support workers and MDT students.
  • Line manage and supervise junior staff within the service.
  • Manage your own caseload, including highly complex cases.
  • Lead and support the team in achieving key performance indicators (KPIs) and maintaining service quality.
  • Foster strong working relationships with statutory, voluntary and independent organisations.
  • Deliver competent and compassionate care to all patients with physical and cognitive difficulties in community and care home settings.
