The goal of the service is to enable patients to make informed choices about how to manage their condition, so that they can stay as well as possible for as long as possible, and reduce their risk of diabetes-related complications. Patients are empowered to self-manage wherever possible.

Main duties of the job

Recent developments, to support evolving health care needs resulting from the remote locality of Powys and Covid 19 pandemic, have included rollout of digital records and video consultations so the location of this post is not fixed.

This a full-time post, but applicants who are looking for part time hours or job shares will be considered.
