An exciting opportunity has arisen at Northamptonshire Healthcare NHS Foundation Trust. We are looking for a Health Care Assistant to join our team. The service aims to provide an excellent standard of person centred planned and unplanned nursing intervention to patients in their homes and is part of an integrated service that supports patients to remain in their home environment. The successful candidate will deliver safe, effective care for the patience experience. As a Health Care Assistant you will work closely with the multi-disciplinary team in order to support the service and have a high focus on delivering quality outcomes and improving the patient experience. As such you will need to be enthusiastic, innovative and highly motivated. Your personal qualities will mean that you have the ability to be flexible to the need of the service and deliver a high standard of care in a supportive and forward thinking environment. The service is provided from 8am to 6pm seven days a week, over a shift pattern. Weekends, bank holidays are a requirement of the job. Car owner/driver and current driving license essential or access to a car (unless you have a disability as defined by the Equality Act 2010)

Main duties of the job

As a Healthcare Assistant , you'll

  • deliver safe, effective care for the patient experience.
  • supports patients to remain in their home environment
  • be professionally accountable for the delivery of care
  • utilise technology to enable you to update records as you go

Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities.

The service aims to provide an excellent standard of care that supports patients. In Northamptonshire you will work as part of a multi-disciplinary team in order to support the service and focus on delivering quality outcomes and improving the patient experience.
