If so, South Tyneside and Sunderland NHS Foundation Trust (STSFT) are looking to recruit highly motivated, enthusiastic and reliable individuals who can work in busy, high-pressured and sometimes challenging clinical environments in the community setting.
We are looking to appoint x 2 full time (37.5hrs per week) Health Care Assistants one to work with our Houghton District Nursing Team and one to work with our West 2 District Nursing Team. The role involves working alone, visiting patients in their own home and providing care in a clinic in our Wound Care Hubs under the direction of a Registered Nurse. The core hours of work are 08:30 - 17:00 across a 7 day week however there may be a requirement to work extended hours dependent on service demands. You must be a car driver and have access to a vehicle.

Training will be available and candidates will be supported with an induction and a personal development plan. Full support will be given to those without community experience to gain the necessary skills required.
It is a condition of employment that all Healthcare Assistants must complete the Care Certificate and achieve their competencies within 14 weeks of commencing in post.
You will work within a Community Integrated Team consisting of District Nursing Staff, Community Practitioners, Specialist Nurses for Older People, Social Services and third sector organisations. You will work under the direction / supervision of a registered nurse delivering high quality nursing care to patients within the community setting including residential homes.
You must also be able to meet the travel requirements of the post and be a car owner and driver.
