Job overview

To provide the support worker role within the Harlow SDFS team based in Stansted.

The successful candidate will work alongside a Multi-Disciplinary Team providing support for people living with dementia and older adults with complex mental health needs.

The support worker role is diverse which can include completing physical health checks, implementing biopsychosocial care plans under the guidance of the qualified MDT staff, building relationships with patients and their families to enable them to live their lives with dignity and positivity.

Main duties of the job

ROLE SUMMARY

  • To work independently and collaberatively as part of a team of Multi Disciplinary  clinicians offering mental health support and treatment in the service area of the Trust. The service will be 5 days a week, Monday to Friday, 9am - 5 pm. the service is delivered in the patients own homes.
  • To act as a role model to other colleagues within the team.
  • Work collaboratively with key partner organisations, service users, clinicians and other practitioners within the multi-disciplinary team.
  • To deliver high quality, effective operational service to people with dementia and mental health needs, in accordance with the principles and objective of the Trust and its vision as identified in the service. The job requires the post holder to work flexibly and collaberatively with all stakeholders within the Integrated Community Team and the wider Integrated Neighbourhood team in Harlow
  • To work collaboratively with others within the organisation to provide an exemplary level of care to service users and their carers.

Detailed job description and main responsibilities

The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our population’s health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a ‘one workforce’ cross system approach to the onboarding, induction, education and experience of Healthcare Assistants.

To support this ‘one workforce model’ the successful candidate for this post will be required to attend our 5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process.

Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience.

This will be followed by an additional 5 day (Monday to Friday) EPUT corporate / mandatory training welcome. (EPUT Welcome will be varied between face to face and virtual delivery).
