An exciting opportunity has arisen has arisen to recruit a Health Care Assistant to work within the at Discharge Lounge at University Hospital North Durham. We strive to provide an effective and efficient service to our patients who require our input in assisting with their discharge home.The Discharge Lounge provides a warm caring environment in which patients can safely wait for discharge home or transfer to an alternative setting such as community hospital within the Trust. Excellent communication skills are essential to ensure a safe discharge and an increase in patient flow around the hospital. The postholder will be required to communicate with the Wards, Doctors, Pharmacy, Relatives, Carers, and Transport and Patient Flow Team. The postholder will collect patients from Wards and departments, collect medication assist with patient care during their time in the discharge lounge and efficient coordination of booking patient transport in readiness for safe discharge.

The successful candidate will give care to patients that they are assigned to under the direct supervision of a 1st Level Registered Nurse, which will include the activities of daily living.

Collect patients from wards and departments.

Collect medication from pharmacy.

Carry out general duties, e.g ensuring all patients have been provided with meals and drinks and organising rubbish in the appropriate bags according to waste management guidelines. Keeping discharge lounge areas tidy and free from clutter.

Answering the telephone giving name and location, directing all patient enquiries to the Discharge Lounge Registered Nurse
