Job overview

The postholder will work as part of the multidisciplinary team and will assist and support the clinical team. This may include working within the patient’s home environment and other clinical settings within community.

The successful candidate will be required to see patients with active and latent tuberculosis in their own homes and at clinics.

They will require unrestricted access to a vehicle to allow for timely travel throughout the Pan Mersey region.

They will work to support the patients as well as having some administrative responsibilities.

A   driving licence  and have use of a vehicle is mandatory for this role to move across sites  within the   community

Main duties of the job

The successful candidate will support the Community TB team. This support can take many forms, such as attending and running clinics, completing administration work,  completing home visits, assessing patients by telephone and much more. The right candidate for this role will be kind, compassionate and sensitive to both colleagues and patients. They will be aware and understanding of different cultures and be able to communicate effectively despite language barriers. They will be required to work flexibly within a small team to provide the best outcomes for the patients. They will be efficient and thorough, with initiative to ensure tasks are completed appropriately. They will be working on a Latent TB screening programme for the most part but will be expected to support the other aspects of the Community TB Service role as required.

Detailed job description and main responsibilities

Work as a member of the multi-disciplinary TB team.

Undertake a range of patient interventions within the scope of NVQ level 3 skills for patients as delegated by team leader or other qualified clinical staff.

Offer advice to patients and carers on a wide range of health issues to include health promotion, lifestyle advice and self-care.

Plan and organise own work schedule once delegated from team leader.

Undertake basic patient observations.

Perform venepuncture autonomously.

Document care clearly and factually.

Work within existing clinical guidelines, policies and infection control procedures.

Offer advice and support to patients and families in a wide range of situations.

Communicate factual and sensitive information within confidentiality guidelines to patients, ensuring all information is appropriately documented and reported back to the team leader.

Demonstrate own role to new starters and students.

Ensure safe moving and handling techniques in the delivery of patient care, this can include the safe use of equipment.

Communicate information to multi-disciplinary team and other agencies as appropriate.

Undertake ordering and receipt of stores.

Undertake further training to gain skills in other areas.

Utilise NVQ level 3 skills within identified areas as required by the clinical team.

Deliver patient care within essence of care standards and trust guidelines and policies.

Document care delivered within hand-written or electronic patient records.

Participate in own PDR with team leader to establish objectives, reporting arrangements, supervision and ongoing development.

Participate in mandatory training events.

Work with community groups to improve patient participation and involvement.
