We are seeking to appoint a dedicated and motivated Health Visitor to work within Belper and Derby City area.

The Belper 0-19 Team have been awarded Gold accreditation for their work with Families and Communities.

DCHS is a forward-thinking, supportive Trust, who are a UNICEF Baby-Friendly accredited organisation and achieved Outstanding CQC status in 2019.

Working in a part rural community you will find that there are lots of opportunities for developing public health work with communities and partner agencies, as well as delivering the core contacts within the Healthy Child Programme.

The right candidates will hold the SCPHN qualification in Health Visiting  and be an innovative and dynamic team players who are looking for a new challenge.

This post is 30 hours per week.

Main duties of the job

  • To provide Health Visiting services to families in the community team caseload by delivering the Healthy Child Programme.
  • To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
  • To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
  • To manage a team of Health Visiting support staff.

  • To manage and take continuing responsibility for a defined caseload/geographical area and be  responsible for the assessment of care needs, the development, implementation and evaluation of  programmes of care and setting of defined standards of care which are evidence based and meet Trust Policies and Procedures to ensure quality of care for patients.
  • Identify, assess and determine client need and where appropriate prescribe from within the nurse prescribing formulary in accordance with evidence, practice-based, patient medical history and Trust Policies and Procedures.
