This vacancy is for 30 hrs per week and the role will involve:
*To provide effective leadership and management to staff, that promotes high performance standards both individually and as a team, in the achievement of the Trust's objectives.
*To identify and assess any children and families in need, and to work in partnership with Primary Care, Children's Centres, Children's Services, Voluntary Sector and other appropriate agencies, to deliver integrated programmes of care within the local area.
*Work within Hampshire Safeguarding Children's Board and Southern Health NHS Foundation Trust (SHFT) policy and procedures, prioritising attendance at Case Conferences, reviews and planning meetings. To be responsible for the provision of case conference reports and court reports as required, contributing to safeguarding of all children.
*To provide a continuum of support according to need, improving outcomes for children by reducing health inequalities. To support the Early Help agenda to ensure early intervention.
*To be aware of the Key Performance Indicators for the Children's Division and focus service delivery towards their achievement.
*To be responsible for the quality and management of the RiO electronic patient record and data reporting system within the team.
*To be the principal point of contact for allocated GP practices and Children's Centres within the area. To develop and deliver integrated working arrangements with these services that support healthy outcomes for children.
