Job overview

The post holder will work within the Primary Care Specialist Nursing Team under the direction of the registered nurse.

The post holder will support Registered Nurses and other health care professionals in the assessment, treatment and management of patients presenting in primary care settings.

The post holder will work as part of the nursing team within standard operating procedures, with Registered Nurses being available for reference.

The post holder will assist with specified delegated clinical tasks which include the provision of diagnostic spirometry testing and monitoring of Chronic Disease management providing a caring, patient focused service at all times.

The post holder will deal with queries from patients, Health Care professionals and other agencies face to face and by telephone.  The role will also involve administration and clerical duties including updating patient records on the computer system.

The role will involve working within a variety of community and primary care settings across Cwm Taf Morgannwg University Health Board (as appropriate to the needs of the service).

Main duties of the job

  • Carry out delegated clinical duties within a variety of clinical settings such as acute hospitals GP practices, mobile units, community clinics and health parks.
  • To perform physiological measurements such as blood pressure monitoring, pulse monitoring, height, waist circumference measurement, weight monitoring.
  • Providing any necessary Health Promotion advice to patients and carers e.g. Smoking Cessation, Weight Management Support, Alcohol Consumption.
  • Undertake Spirometry testing including the administration of salbutamol aerosol in accordance with ARTP standards and local PSD requirements and record results in patient’s records.
  • Undertake FeNo and other respiratory diagnostic testing in accordance with local and National guidance and record results in the patient’s record.
