We have an exciting opportunity for a Health Care Support Worker to join our welcoming community District Nursing team in Uttoxeter. This is a full time, permanent role based at Balance st Surgery in Uttoxeter, covering the Uttoxeter and surrounding villages.  We are a passionate team who thrives to improve quality of Health care for our patients.

You will be part of a team striving to achieve the best in clinical care. The service will deliver a pathway of care in accordance with the latest evidence-base, which puts the service users needs at the centre of the service.

We cover a 7-day service 08:30 - 17.00hrs. Due to the nature of community working, ability to travel across the patch to provide care to patients is essential; access to a car during working hours is required.

Main duties of the job

To adhere to the Care Certificate standards and all other relevant guidance from the Partnership Trust. To be aware of the Nursing & Midwifery Council’s (NMC) guidance/codes of conduct.

To establish and maintain effective working relationships within the Partnership Trust, with external organisations and our patients in order to meet service requirements and support and demonstrate the Trust’s equality values.

To contribute to to the treatment and care for individuals, families and carers under the guidance of a registered practitioner. The care/support plans may include the following:

  • Promoting independence/self management and maintaining safety
  • Diabetic care- including insulin administration  and blood glucose monitoring
  • Medication administration- injections
  • Wound care
  • Moving & Handling- including moving & handling patients
  • Observations, venepuncture
  • Catheter management
  • Admission avoidance
  • Specific skills as listed in person specification

All under the guidance of the registered nursing team.

To consider each service user’s personal circumstances and preferences in order to implement and adjust programmes of care and intervention.

To maintain and develop skills and knowledge to deliver a range of interventions under the guidance of a registered practitioner and set an example of good practice to other team members including skill sharing.
