Job summary
We are looking to recruit an enthusiastic Health Care Assistant who has a genuine interested in providing safe and effective care for patients at home. The successful candidate must have demonstratable experience, a wide range of clinical skills and will be confident to work without direct supervision. Community nursing is evolving at a fast pace, and you must be able to engage positively with service improvements and change.

Main duties of the job
The role requires the staff member to work as part of a Community Nursing Service to deliver high-quality care to patients in their own homes in accordance with individual care plans. Provide personal and palliative care, ensuring patients are treated with dignity, respect, and compassion while supporting their comfort and wellbeing. Demonstrate the ability to work independently and autonomously within the community setting, effectively manage responsibilities while maintaining a high standard of care. Support the Community Nursing Team by managing the ordering and organisation of clinical supplies and stationery to ensure services operated smoothly. Consistently follow Trust policies, procedures, and professional standards in all aspects of work while maintaining strict adherence to health and safety regulations, infection prevention, and control guidelines. Ensure patient confidentiality is always maintained and information is handled sensitively and responsibly in line with legal, ethical, and organisational requirements. Contribute to a safe, supportive, and professional care environment for patients receiving community-based nursing services.
