Job summary

We have an exiting opportunity to join us as a Health and Wellbeing Coach.

Gosport Central Primary Care Network are looking for a person that can motivate patients to manage their own health & well-being to live independently and improve their health outcomes. The post holder will work with individuals and/or groups of patients to deliver sessions on how to motivate patients to improve their physical activity, eat healthily to meet personalised care plans.

You will use a holistic approach to promote lifestyle changes, support patients to address their existing health problems and prevent new ones.

The role works within their practices multi-disciplinary team, a full induction and training is given to help candidates integrate into Primary Care.

Main duties of the job

As a Health and Wellbeing Coach, you will work with patients to support positive behaviour change and improve their overall health and wellbeing. You will provide personalised coaching to help individuals manage long-term conditions, set and achieve health goals, and build the confidence and skills needed to take greater control of their health. Working as part of the Primary Care Network multidisciplinary team, you will help address wider determinants of health, connect patients with appropriate community and support services, and contribute to reducing health inequalities and improving patient outcomes through a person-centred approach.
