We have an exciting new role for a GP to join our interdisciplinary, integrated community team providing more acute care for patients in their own homes. This is an opportunity to expand and develop your portfolio career.

This roles are for 4 x sessions, one role to cover Wednesday and Friday and one role to cover Thursday and Friday.

This role will be a new role for the department. Providing more acute care for patients safely and conveniently in their own homes is at the forefront of national frailty service design. The successful applicant will primarily be working within our developing  virtual ward model with a specific role supporting GPs to access the right care at the right time for their patients and effectively utilise our growing community based services as an alternative to hospital admission.

We work very closely with the ARCH team (Acute response and rehabilitation in community and hospital) which includes occupational therapists, physiotherapists, nurses and multidisciplinary assistants.

We also have a team of unscheduled care nurses and UCR practitioners who support delivery of the service across the HaRD area.

  • To work as part of the multi-disciplinary team in order to support and help to provide a comprehensive Community Geriatric Service within the Trust
  • Maintenance of the highest clinical standards in the management of patients.
  • Assisting with teaching, training and assessment of UCR staff and other members of the multi-disciplinary team
  • To actively participate in both departmental and Trust matters concerning Clinical Governance and audit.
  • To have responsibility for ensuring active participation in continuing medical education (CME)
